Having such knowledge, I hereby acknowledge this release and release any representatives, agents, and Successors from liability for accidental injury or illness that may incur because of participating in the said program. I agree to disclose any physical limitation, disability, ailment, or impairment that may affect my ability to participate in said fitness program.
